Digital Approach: Predictable Outcomes in Minimally Invasive Dentistry

Esthetic full-mouth rehabilitation of severely compromised dentition represents a great challenge for clinicians and dental technicians. Workflows that improve the predictability of the final treatment outcome of these complex cases are therefore needed. Advancements in CAD/CAM material systems, digital restoration planning and fabrication enable the development of innovative treatment concepts for the severely compromised dentition. Digital face scans and novel CAD/CAM applications offer a standardized manufacturing process, resulting in a reliable, predictable and economic workflow for complex teeth supported restorations. Modified onlay and full veneer preparation geometries evolved and form a reliable alternative for conventional full-coverage crowns. Nowadays non-retentive preparation designs in combination with reduced ceramic thicknesses allow for a minimal invasive treatment approach. At the conclusion of this lecture, participants will be able to identify: 1) The guidelines for minimal invasive preparation designs and recommended ceramic thicknesses. 2) Rationale of case based selection of modern ceramic materials and CAD/CAM systems. 3) Case series and the scientific evidence pertaining to the clinical success of CAD/CAM materials and digital restorative technologies.

The Prosthetic Revolution
A myriad of factors affect the esthetic and functional outcome of complex cases. Properly addressing those factors will facilitate the achievement of a predictable and successful prosthetic rehabilitation. Among others, factors that will significantly improve the functional and esthetic outcome include: proper preoperative esthetic and functional analysis, correct data transmission to the laboratory regarding the occlusal plane orientation and inclination, and definition of an appropriate incisal edge position. A close collaboration is also needed between the surgeon and the restorative dentist both for treating natural dentition or dental implants, especially in the anterior area, in challenging clinical situations such as patients with a high smile line. Today the goal is to switch from the traditional to the digital workflow starting with the support of a specific App for the treatment plan, continuing with the use of 3D intra oral scanner for the impression, and finishing with CAD/CAM fabrication of the restorations. Material selection plays a fundamental role in management of complex rehabilitation cases. This presentation will also illustrate how to select and optimize the use of metal-free ceramic materials even in full-mouth rehabilitation. Nowadays an innovative operative protocol allows to face highly compromised clinical situations, with a minimally invasive prosthetic procedure (MIPP). Thanks to this technique it is possible to achieve excellent esthetic results using veneers, full-veneers, buccal-occlusal veneers and table tops even with very reduced ceramic thickness. The maintenance of a maximum amount of enamel and the use of the appropriate bonding technique guarantee a remarkable resistance and a long-lasting result to the restorations. At the conclusion of this lecture, participants will be able to: 1) Perform an ideal esthetic and functional treatment plan with a digital approach. 2) Select the appropriate technique and ceramic material when approaching a full-mouth rehabilitation. 3) Identify innovative operative protocols with minimally invasive prosthetic procedures (MIPP) for a long-lasting esthetic result.

Team South America: The Other Face of Esthetics: Integrating Function into Smile Design
The Esthetic Functional Integration has always been one of the major objectives and challenges of a dental rehabilitation. The goal of this lecture is to share our learning curve on the process of becoming a modern clinician/technician, Smile Designer and Treatment Planner. The process of designing a smile that combines esthetics and function evolved from a disciplinary process into an interdisciplinary one. It also moved from an afterthought process to something planned from inception. The philosophy of a Facially Driven treatment plan expresses exactly that. That is why we work first on a facially driven smile design project before Brainstorming the treatment plan and developing the negotiation between Biology, Function and Structure into the Esthetic project. One of the challenges of the smile design process has always been the link between the face and the working model. As a technician I always had the feeling that I was developing wax-ups “lost in space”. Even when working with articulators and face-bows, the facial esthetic integration was never totally transferred to the technician, generating excessive intra oral adjustments for the dentist to improve facial harmony. Digital dentistry came to help us in this process, creating a link between the patients face and the lab design. The evolution of this process and the trends for the future will be presented, moving us from analogue to digital and from 2D to 3D. The integration of motion in the process of designing esthetics and function is also vital. From simple smartphone videos to 3D facial scanners with movement, Facial and lip dynamic analysis and jaw motion are now part of the process. Developing naturally looking smiles is also a great challenge for smile designer. The new concept of copying nature through scanners and using natural algorithms is a reality that will change the restorative world for better, simplifying complex handmade procedures with a complete digital workflow. Customizing smiles and integrating them, ethically, with function, biology, facial and emotional aspects of the patient is our ultimate goal. At the conclusion of this lecture, participants will understand: 1) The step-by-step of the Esthetic Functional Integration. 2) The complete digital workflow implementation. 3) How to perform facially driven CAD/CAM restorations.
