Team Southeast: The Role of TEAM in Reconstructive Esthetic Dentistry: A Defined Algorithm for Success
With the advent of new technologies, techniques and materials the ability to replace missing teeth with the form, function and beauty of the natural dentition is now a reasonable possibility. All ceramic restorations as well as implants can be offered as solutions to patients suffering from the loss of teeth in an esthetic world. With any and all of these procedures, in order to attain a successful outcome, the focus must be the framing of the restoration within the confines of the face, lips and gingival (even artificial gingival) to provide for ideal proportion and beauty in the final restoration. Therefore, any restorative endeavor in this situation must entail the foundation of a beautiful soft tissue frame and the construction of harmony of the gingival scallop between the restorations, the adjacent dentition, the border of the lip perimeter or between the hybrid restoration and facial interfaces. This presentation will focus on a defined algorithm for the interdisciplinary team of the restorative dentist, periodontist, orthodontist, lab and radiographic technicians to successfully reconstruct the soft tissue foundation for all of these restorative options, as well as the interaction required for 3D planning of the full arch patient in need of artificial gingiva. The diagnosis of deficiencies, as well as the varied treatment options, will be discussed. This includes bioengineering, regeneration of hard and soft tissues, as well as 3D planning and esthetic framing. At the conclusion of this lecture, participants will be able to identify: 1) The risk factors in anterior implant therapy. 2) How and when to successfully incorporate minimally invasive protocols. 3) How the new 3D treatment planning effects clinical accuracy and outcomes. 4) How the new Digital and CAD/CAM technology optimizes implant therapy.

Contemporary Orthodontics: A Transaction, or Transformation?
In dentistry, the goals of treatment are often designed for what are patients wish for, even demand. We all see it - instant satisfaction, immediate outcomes. But when we are anticipating treatment for an adolescent, whether it be strictly orthodontics or including interdisciplinary care, we must often take into account the hard and soft tissue changes that occur over the lifetime of that patient. In other words, we commit ourselves to designing treatment with a longer-term view. In this presentation, Dr. Sarver will cover the principles and will present newly gathered data on how smiles change with time and how we adapt our treatment. In addition, we will take a very comprehensive look at facial appearance and its importance to the immediate and long-term needs of the patient. At the conclusion of this lecture participants will be able to or have gained an understanding of: 1) Craniofacial growth, maturational and aging changes. 2) Soft tissue growth, maturational and aging changes. 3) How the two factors interact and influence our esthetic plans.

Interdisciplinary Interactions to Enhance the Outcome of Prosthodontic Treatment
A number of interdisciplinary procedures that can enhance the outcome of prosthodontic treatment are available to clinicians; from forced eruption of single tooth to extensive orthodontic treatments; from muco gingival periodontal procedures to more extended surgical options. Advantages and shortcomings of interdisciplinary treatments need to be evaluated and often compared to possible alternative simplified implant procedures. The alternative to forced eruption to recover a single tooth may be an implant, the alternative to a complex situation maybe a full arch immediate loading. The principle of preserving teeth needs to be confronted with the complexity and prognosis of treatments. A good knowledge of possible interactions with other disciplines should be mandatory to the prosthodontist to plan when and how to incorporate an interdisciplinary treatment approach into everyday treatment and to be able to lead an interdisciplinary treatment plan. Unfortunately, quite often interdisciplinary treatment management may be frustrating and the interaction between different specialties may lead to additional problems instead of additional solutions. “Fuzzy” exposure to even the highest state of the art treatment in each discipline may result in enormous waste of time and money with far from ideal results. Optimal multidisciplinary or interdisciplinary management should maximize treatment results by synergizing the knowledge, skills and possibilities of all different dental disciplines, and at the same time minimizing frustrations and shortcomings that may occur in treatment management. The pros and cons of various treatment options will be reviewed and discussed. At the conclusion of this lecture, participants will be able to: 1) Identify possible interdisciplinary interactions that can enhance the outcome of prosthodontic treatment. 2) Identify treatment sequence in interdisciplinary treatments. 3) Lead an interdisciplinary team.

Prospective Fellow Presentation: Lip Dynamics Revisited
Decisions about tooth position and the influence of gingival symmetry and scallop are generated from the diagnostic records that document tooth and gingival display. These diagnostic records often include photographic or video documentation. The metrics provided by the social smile, a commonly utilized diagnostic record, often prove to be inaccurate. The well-meaning clinician can find themselves post treatment realizing the influence of the gingival symmetry and scallop has greater impact on esthetics than originally perceived. Hence the move to video capture in an effort to circumvent the limitations of lip dynamics seen with the social smile. This presentation will discuss photographic vs. video documentation and the preferred techniques for capturing maximum lip dynamics. At the conclusion of this lecture, participants will be able to identify: 1) The lip movements that best capture maximum maxillary and mandibular tooth and gingival display. 2) Efficient methods for capturing quantifying tooth and gingival display.

Predicting Clinical Performance of Dental Composites Through In Vitro Assessments: Is It Possible?
The diversity and continued development of dental composite materials, along with their rapid introduction to the dental community, ensures that clinical studies demonstrating efficacy and effectiveness will not always be available. This puts a great burden on in vitro assessments of the properties and characteristics of these materials to attempt to predict clinical performance. Is this realistic? The published in vitro work on dental composites represents a massive volume of broad and extensive research. Surprisingly, there appears to be only minimal evidence of direct correlations between the properties we believe to be important to ensure clinical success, and the actual demonstration of that relationship. There are standard tests for assessing certain properties of dental composites, but these were only intended to guarantee some level of safety and were not meant to predict clinical performance. Considering the complex situation of the oral environment and the demands, both physical and esthetic, on dental composites, it is critical to identify the most important characteristics of the materials that can be assessed in vitro for the purpose of predicting clinical performance. The current state of the knowledge in this area will be addressed in this presentation, and recommendations made. At the conclusion of this lecture, participants will be able to: 1) Identify the composition of the many types of dental composites, and explain how their properties are influence by their composition. 2) Compare the properties of dental composites to those of other dental restorative materials that have experienced clinical success. 3) Describe the in vitro testing methods that are expected to be the most influential for assessing the physical and esthetic properties of dental composites, and that they may be expected to show a correlation with clinical performance.

Composite vs. Ceramic: Interdisciplinary Team Protocols
The ever-increasing demand for functional and esthetic materials is leading companies to continuous evolution and research. Indications about full ceramic or composite restorations are becoming ever less absolute, considering many clinical aspects you can opt for a material rather than the other. The versatility of the composite resin, its possibility of layering intraorally, the ease of repair and the fairly good prognosis in the medium and long period, make it a reliable material. The tendency of new materials to improve the wear resistance and stability, in many cases, is a worthy substitute of ceramic materials. By contrast, ceramic materials, often considered the gold standard for color characteristics and mechanical and physical properties, see an increase in the ability to produce restorations with minimum thicknesses while maintaining dimensional stability, prognosis and esthetics. The choice, therefore, to use ceramic or composite materials is always less obvious, but, on the contrary, offers the dentists greater possibility of diversification in restorative. Finally, many aspects could be discussed, such as: indications, mechanical characteristics, esthetics, predictability and durability. In order to achieve an optimal esthetic outcome, an integrated approach to treatment is required to reconstruct the ridge and replace the missing teeth. This presentation will review the treatment planning, sequencing, and management of alveolar ridge deficiencies utilizing an interdisciplinary approach.
