Management of Critical and Sub-Critical Contours on Implant and Tooth Supported Restorations The relationship between restorative contours and gingival tissues has often been debated with limited conclusive evidence. Current trends generally advocate the use of undersized contours to achieve soft tissue thickness. That this approach can be universally applied without the sequelae of gingival margin instability, inflammation and collapse of soft tissue profiles, remains to be demonstrated. This presentation will review variables that may influence contour management in the critical and sub-critical areas, and provide guidelines for achieving predictable outcomes. At the conclusion of this lecture participants will be able to: 1. Concept of critical contour and its relationship to clinical crown morphology. 2. Sub-critical contour management in tooth supported vs. implant supported restorations. 3. Effect of tooth and implant position on restorative contours.

The use of short implants in the atrophic posterior mandible The atrophic posterior mandible presents unique challenges for dental implant replacement. Traditionally when inadequate available bone was present superior to the mandibular canal for implant placement (< 10.0 mm), either nerve repositioning or vertical bone augmentation was performed. As an alternative to this approach, short implants have been utilized to reduce morbidity, costs and treatment length. Many studies are finding the prognosis of short implants is comparable to those of longer implants placed vertically in augmented mandibles. At the conclusion of this lecture participants will be able to: 1. Understand the anatomic limitations of implant placement in the posterior mandible 2. Counterpoint - Treatment of the Posterior Mandible: Short Implants or Vertical Tissue Augmentation?
Management of Soft Tissues in the Vertical Bone Augmentation The dental implant therapy associated with vertical bone defects, often sets limits to the clinical practicality. The GBR technique, using e-PTFE reinforced membranes, is one of the surgical techniques that allows the clinical to treat these kinds of bone defect. Despite the fact that clinical evidence shows that GBR is within a wide span of time, a successful technique, there are many practitioners and authors who claim that this technique is operator’s skills sensitive. It is often asserted that the surgical predictability relies almost exclusively on the accurate management of soft tissues which is not always affordable when regenerating large amounts of bone tissues. The share of soft tissue advanced coronally not always permits the clinician to join the flaps over the membrane in a fully passive way therefore, causing a possible early exposure of the membrane and the subsequent failure of the technique. The introduction of two new surgical techniques of the management of the lingual and buccal flap improved the reproducibility and validity of the GBR procedure, enhancing the surgical predictability of this technique.The new management of the soft tissues, good competences, the introduction of the concept volumetric encumbrance, and a needed learning curve are changing the surgical and mental GBR approach and maybe we could consider it as a technique where the important thing is the knowledge of the clinician rather then his skills. Soft Tissue Procedures Around Dental Implants: Why, When, and How; The Role of Connective Tissue Grafting Around Teeth
While well-established protocols result in high implant functional success, newer treatment strategies aimed at improving soft tissue stability, contour and color have developed from an increasing demand placed on esthetic outcomes. Although early implant literature suggested that attached tissue was not essential for the success of implants, today the esthetic and comfort value of keratinized and attached gingiva has been widely accepted. Similarly there is a clear understanding that esthetic treatment outcomes are affected by surgical and transitional restorative techniques, bone volume, and the 3 dimensional relationship between bone/implant position/gingival position. As a result, the goal of long-term soft tissue stability and protection of underlying bone has increased the use of autogenous grafts and acellular dermal graft materials to enhance tissue contour and quality around implants. This session will explore current evidence-based management of soft tissues around implants during the surgical and prosthetic phases of treatment. At the conclusion of this lecture participants will have gained an understanding of: 1. The zone of keratinized tissue and volume of tissue around an implant: what are the driving features that indicate a need for augmentation. 2. Autogenous tissue (thin/thick, various harvesting sites), allografts and other: does the graft source influence outcomes. 3. Adjunctive non-surgical procedures to enhance tissue characteristics and volume around implants: when are they indicated ____________________________________________________________ This presentation will focus on the use of autogenous soft tissue grafts as a strategy for root coverage. We will discuss surgical manipulation of the donor and recipient sites as well as potential surgical and healing complications. Back to the Basics: Proper Occlusion to Rehabilitate Chronically and Cognitively Impaired Patients
Relationship between mastication, total health and brain functions have been studied and identified in several publications. My practice includes many elderly patients with cognitive impairment who have been unable to function due to ill fitting dentures. It has been my clinical experience that carefully designed dentures and proper adjustment of the existing dentures help improve the masticatory functions, which in turn improved cognitive capacities. Long-term illustrative cases will be presented with video recordings showing the remarkable effects of the well functioning prostheses on the quality of life of the individuals.
