Restorative Materials in Fixed Prosthodontics: Not every restoration should be an all-ceramic one
A restoration's esthetic success depends on the ability of the technician to reproduce not only the natural tooth's color, but also its correct translucency. In the recent years, the indications for the use of all ceramic restorations have expanded also thanks to the increase in the range of opacities/translucencies of many metal-free materials. Especially when dealing with highly translucent teeth, the color of the underlying tooth structure or preprosthetic restoration may have a negative impact on the esthetic outcome if the prosthetic material is not able to mediate or hide dark substrates while providing the proper degree of light transmission/reflection. This lecture will illustrate the considerations that influence optical behavior of different all ceramic materials and provide some recommendations when dealing with dark abutments. At the conclusion of this lecture participants will have gained an understanding of: 1. The differences among all ceramic materials as far as their rang of opacities/translucencies is concerned 2. Enhancing the Strength and Esthetics of Zirconia to Achieve Restorative Success
Full-contour zirconia restorations are gaining popularity among clinicians due to favorable biological and mechanical properties. Initial monolithic zirconia restorations were opaque and, therefore, limited to the posterior region. In recent years, manufacturers have increased the translucency of zirconia. Creating a zirconia material that provides both strength and beauty is achieved through various manufacturing methods, and by determination of an ideal thickness of the restoration. This presentation discusses both flexural strength and translucency of all-ceramic, resin-ceramic and silica-based ceramics available today at varying thicknesses. Independent research is presented that supports the use of highly translucent zirconia in various anterior and posterior cases, noting key factors to predict success. CAD/CAM Vs. Traditional Ceramic Techniques in Restoration Fabrication
New technologies are constantly changing clinical and laboratory protocols and workflows. Knowledge of CAD/CAM software, production technology, CAD/CAM materials and handling properties as well as the combination and application of different ceramic materials are fundamental to the clinical success and longevity of the restoration. To take full advantage of the computerized technology, a perfect digital workflow has to be established between the dental technician and the dentist. However, even the most sophisticated digital system has its limitations and a combination of traditional manufacturing and CAD/CAM fabrication is advised to ensure an efficient workflow. This presentation discusses possibilities and limitations of CAD/CAM-fabricated restorations in comparison to traditional ceramic techniques. Furthermore, it will outline how to establish an efficient and predictable digital workflow between laboratory and dentist and how to implement, when necessary, traditional restoration techniques into the digital workflow. Expanding Horizons in Adhesive Dentistry
Adhesion has completely transformed the way that Restorative Dentistry is performed today. Novel minimally-invasive and highly conservative bonding techniques allow preservation while reinforcing dental structures being considered as the main approach for restorative treatment whenever it is possible. However, producing successful and satisfying bonded restorations is still dependent on numerous factors, especially with respect to material selection and bonding integrity. Impairment of the adhesive interface can produce catastrophic failures that can be avoided if the adhesive technique is adequate. Superior longevity can be achieved by improving the bonding effectiveness through reliable bonding methods and materials. This presentation provides a systematic and scientific approach for enhancing bonded esthetic restorations based on original research data with special emphasis on techniques, post-operative sensitivity control, and new materials design and selection. Risk Indicators and Prevention of Mucosal Recessions
Soft tissue recession around dental implants has become a major concern among many implantologists. This lecture addresses the etiologies as well as the approaches to avoid this problem before they occur. An ideal implant 6-dimenisonal positioning to avoid this problem will be demonstrated. Research related to soft tissue recession around dental implants will be analyzed and discussed. The pros and cons of techniques used to treat soft tissue recession around dental implants such as various soft tissue grafts (e.g., connective tissue graft, acellular dermal matrix, free soft tissue graft, guided bone augmentation), as well as implant removal will be discussed. Extrude or Extract: How Do You Decide
Since the introduction of implants into the US in the early 1980’s, and with their high levels of survival, it has become easier and easier to condemn “questionable teeth” for extraction. Yet virtually every clinician has horror stories of significant complications arising following tooth removal and implant placement. This presentation will look at an old, but often overlooked modality for maintaining questionable teeth, orthodontic extrusion. The presentation will provide a systematic approach to evaluate if extrusion is a reasonable alternative to extraction.
